A Patent License is not a form for the novice. If you've got a patent, go get an attorney to do your patent licensing. However, for pedagogic purposes you can have a look at an example of a "short" Patent License Agreement.
LICENSE AGREEMENT
This agreement, numbering five (5) pages, is entered this ___ day of ______________, 200__, by and between _________ , an individual residing at ________, ____________________, hereinafter referred to as the LICENSOR, and ________________________ a ___________ Corporation, and having its principal place of business at 5175 _____________________________________, hereinafter referred to as the LICENSEE.
RECITALS
WHEREAS, the LICENSOR is the inventor of certain inventions and improvements described in Exhibit A, and the LICENSOR has represented that he can and will obtain a valid U.S. Patent for the invention described in Exhibit A, hereinafter referred to as the PATENT or INVENTION;
